Obeticholic Acid—A Pharmacological and Clinical Review

Obeticholic acid (OCA) or 6-alpha-ethyl-chenodeoxycholic is a semisynthetic modified bile derivative that acts on the farnesoid X receptor (FXR) as an agonist with higher potency than acid. The FXR nuclear highly expressed in liver and small intestine regulates acid, cholesterol, glucose metabolism, inflammation, apoptosis. group of receptors currently under investigation for their potential role treatment primary biliary cirrhosis (PBC), non-alcoholic steatohepatitis (NASH), sclerosing cholangitis (PSC). Recent clinical studies suggest OCA may work synergistically lipid modifying medications to further improve long-term outcomes cholangitis. Specifically, can NASH patients different histological, metabolic, biochemical issues well morbidity mortality suffering from PBC, PSC, disease. This improvement noted both improved histological examination reduced need transplantation. In this review, we examine pharmacology towards PBC refractory (NASH). addition, future directions applications NASH, NAFLD.
